VA inspector general's Pittsburgh inspection finds cleanliness, equipment-testing and electrical-cord fixes needed

The VA Office of Inspector General (OIG) published its Healthcare Facility Inspection of the VA Pittsburgh Healthcare System on Tuesday, Oct. 6, 2026. These inspections are routine, focused check-ups the OIG runs at VA health systems around the country, not a response to a single complaint.

The inspectors looked at five areas: the facility's culture, its environment of care, patient safety, integrated veteran care and what the OIG calls the veteran-centered safety net.

The OIG made four recommendations, all in one area, the environment of care: keeping patient care areas and storerooms clean; making sure medical equipment is inspected and tested on schedule; handling pharmaceutical waste containers correctly; and power strip and electrical cord safety. In other words, the kind of housekeeping a hospital is supposed to sweat, literally and figuratively.

The other four areas drew no recommendations in this report. VA facilities respond to OIG recommendations with action plans, and the OIG tracks them until they are closed.
