Proposed Extension of Information Collection: Certification and Qualification To Examine, Test, Operate Hoists and Perform Other Duties

MSHA seeks comments to November 30, 2026, on renewing the hoist-operator and mine-examiner certification paperwork (OMB 1219-0127, Form 5000-41). FR 2026-19837.

Hoist operators and the people who examine and test mine equipment need proper certification, and the Mine Safety and Health Administration keeps track of it on a federal form. MSHA wants to keep that paperwork going and is taking comments until November 30, 2026.

The collection is titled “Certification and Qualification to Examine, Test, Operate Hoists and Perform Other Duties,” OMB Control Number 1219-0127. It uses MSHA Form 5000-41, the Safety and Health Activity Certification or Hoisting Engineers Qualification Request Form.

MSHA’s yearly estimate: 867 respondents, 3,818 responses, 351 hours of total time, and $31 in recordkeeping costs. It is an extension without change.
